I look forward to providing personalized care for you. Please note that my office doesn’t bill insurance directly, which allows me to provide you with the best treatment you need, rather than what insurance dictates.
Once you’ve paid for your appointment, my office will provide a detailed invoice that you can submit to your insurance. If you have PPO insurance, you may be reimbursed for the portion they cover for out-of-network care. If you have any questions about the claims process, we’re always happy to help. You may also contact your healthcare insurance company directly to inquire about reimbursement for services (CPT codes) listed below.

Fee Schedule
Adult Diagnostic Evaluation (CPT code 90792) $400
Child/Teen Diagnostic Eval (CPT code 90792 x2) $800 includes 1-hour appointment with child/teen and additional 1-hour session with parents for minimum 2 appointments @ $400 each.
Complex cases may require more than the minimum listed initial appointments for thorough evaluation
Interactive Complexity add-on (CPT 90785) $25 in addition to baseline evaluation fee above, added for evaluations that require communication with other individuals legally responsible for their care, such as minors or adults with guardians, adults accompanied by a family members or interpreter, and any other circumstances that add to complexity of evaluation.
Follow-up sessions for ongoing treatment are billed as follows:
30-min session $230
(=$130 med management CPT 99214 + $100 psychotherapy CPT 90833)
45-min session $300
(=$130 med management CPT 99214 + $170 psychotherapy CPT code 90836)
60-min session $370
(=$130 med management CPT 99214 + $240 psychotherapy CPT 90838)
Interactive Complexity add-on $25 (CPT 90785) in addition to baseline fee above, added for complexity in communication
Ketamine Session Fees
$680 for 2-hour ketamine journey with Psychedelic Facilitator +45min psychotherapy with Dr Rai = $325+355
Or $750 for 2-hour ketamine journey with Psychedelic Facilitator +60min psychotherapy with Dr Rai = $325+425
Or $1200 for 2-hour ketamine journey with Dr Rai +60min psychotherapy = $800+400.
If longer sessions are required you are billed accordingly.

2-hour Ketamine journey with Psychedelic Facilitator $325 (CPT H0033)
2-hour Ketamine journey with Dr Rai $800
($400 prolonged service CPT 99415 + $200x2=400 CPT 99416 for 2 additional 30-min periods)
Psychotherapy (billed in addition to 2-hour session). Each KAP session includes at least 45 min time with Dr Rai in addition to 2-hour ketamine journey, typically split into 15 min before ketamine journey and 30 min after. If you need more time, you are billed for length of session you require.
45-min Psychotherapy $355
(=$160 med management CPT 99214 + $170 psychotherapy CPT 90836 + $25 Interactive Complexity)
60-min Psychotherapy $425
(=$160 med management CPT 99215 + $240 psychotherapy CPT 90838 + $25 Interactive Complexity CPT 90785)
Medicine (ketamine) cost is approximately $150-175 for 4-week supply, and paid directly to pharmacy, not to Dr Rai’s office
Breathwork Session (no Ketamine) 1-hour $200
Learning effective ways to use breath to regulate arousal in body can be very effective to prepare for ketamine sessions and also help in integration phase by enabling one to journey on music and breath without medicine once ketamine sessions are discontinued.
Payments
Payment for treatment cash, check or credit/debit card is due at the time of your visit.
All patients are required to have a valid credit card on file.
If you need to cancel or change your appointment, please call/email my office at least 2 business days before the appointment, otherwise your card will be charged the full fee for the appointment reserved exclusively for you.
Good Faith Estimate of Treatment Costs
As of 2022, a new law requires that I offer you a “Good Faith Estimate” explaining how much your mental health care will cost. Under the law, health care providers (including mental health clinicians) need to give patients who don’t have or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. This is based on the No Surprises Act of 2020 and was originally designed with hospitals and bigger treatment facilities in mind, where you may be surprised by unexpected high costs of surgery or hospitalization or other treatment.
Unlike a surgical procedure or other discrete medical services, it can be difficult to estimate the total cost of therapy services because this number is influenced by length, frequency and type of sessions you need. However, if you would like a Good Faith Estimate, based on once or twice weekly visits per month for 30, 45 or 60-min appointments, my office staff can provide one for you.
